This time,Visited 6 leading hospitals in India (400-1,500 beds) to observe the introduction of PICC (Peripherally Inserted Central Venous Catheter)The first two hospitals in the country were In these hospitals,10-15 PICC insertions are performed per day, and on-site surveys are conducted on the implementation methods and operational systems at each facility.The participants also gave a presentation on the current status of PICC use and its operation at their own facility (Kyoto Katsura Hospital). RGCIRC (Rajiv Gandhi Cancer Institute & Research Center)

RGCIRC is one of the largest private, not-for-profit hospitals dedicated to cancer care in Delhi, the capital of India, with over 500 beds. Although not a public hospital, it is funded by government and charitable organizations to provide multidisciplinary cancer care without being influenced by economic constraints.
Nurses also play a very important role. A system for PICC insertion is in place, and nurses themselves are primarily responsible for the insertion and management of PICCs, including ultrasound-guided puncture.

MEDANTA Hospital

MEDANTA Hospital, located in Gurgaon, a city southwest of Delhi that has grown rapidly in recent years, is one of India's leading general hospitals, founded by cardiac surgeon Dr. Naresh Trehan. It offers state-of-the-art medical facilities and highly specialized care, and has been named one of the "Best Hospitals in the World" by Newsweek magazine.

The hospital has an active conference and educational activities by medical professionals and is highly regarded both nationally and internationally. With a large number of physicians, PICC is mainly inserted by radiologists and intensivists.

TMC (Tata Memorial Center)

Tata Memorial Center (TMC), located in Mumbai, India's largest commercial city, is a public hospital founded by the Tata conglomerate, one of India's largest conglomerates, and currently run by the government. 650 beds and over 50,000 cancer patients treated annually, TMC is one of India's It is one of the best cancer hospitals in India, with 650 beds and more than 50,000 cancer patients treated annually. Being a public hospital, the number of patients is very high and the facilities are limited in terms of aesthetics and equipment.

Although nurses play a major role, echo-assisted puncture is not performed, and the environment for PICC insertion is not well-developed.

RIMC Chennai (Dr.Rela Institute & Medical Center)

Chennai is the largest city in South India, where traditional culture meets modern industry. RELA Institute & Medical Center here is a for-profit private hospital specializing in critical care and organ transplantation and is one of the largest transplant performing hospitals in India.It has 450 beds and was founded in 2018 by liver transplant surgeon Dr. Mohamed Rela It was founded in 2018 by Dr. Mohamed Rela, a liver transplant surgeon.

Many patients from outside of Japan come to the hospital for transplant care, and the PICC is inserted primarily by a physician under echo-guided guidance.

CMC Kannigapuram(Christian Medical College Vellore, Ranipet Campus)

CMC Kannigapuram, located in Ranipet, a city near Chennai that has developed as an industrial area, is a for-profit and non-profit private, general hospital serving a wide range of disciplines, including trauma.

With 1,500 beds, a highly specialized nursing staff, and a good understanding of physicians, the environment for PICC insertion is the most well-developed, and nurses are able to perform ultrasound-guided punctures. Nurses are primarily responsible for PICC insertion and management

TMC (Tata Medical Center)

Located in Kolkata, the largest city in Eastern India, Tata Medical Center is a private, for-profit and non-profit hospital established in 2011 with 431 beds and is considered the best cancer treatment facility in Eastern India. Although not a public hospital, it provides multidisciplinary cancer care without financial constraints, thanks to funding from TATA conglomerates and charitable organizations.

Although nurses play a major role, they blindly puncture without using echo, and the environment for PICC insertion is far from adequate.

Who puts in the PICC and where?

Critically ill patients have the PICC inserted by an intensivist in the ICU, while general patients are handled by a radiologist or nurse practitioner, depending on the hospital. In many cases, the radiologists are in the angiography room and the nurses have a PICC room and do not insert at the bedside. In private hospitals, most insertions are performed by radiologists, and pay is based on the number of insertions, as it is a pay-for-performance system.

In hospitals where nurses primarily inserted PICCs, a system was in place where nurses not only inserted PICCs, but also set up pre-interview rooms and post-insertion management rooms (including management of outpatients) to take care of pre- and post-PICC management. Nurses are good at managing the entire patient population.
